We describe the case summary of a 70-year-old man diagnosed with interstitial lung disease due to prolonged nitrofurantoin therapy. Despite honeycombing confirmed by computed tomography of the thorax, symptoms and radiographic findings disappeared within 1 month after withdrawal of nitrofurantoin. 504633/PL1b ® MacroBID 100 mg prolonged-release capsules (nitrofurantoin) Patient Information Leaflet Read all of this leaflet carefully before you start Use this medicine for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antibiotics. Nitrofurantoin will not treat a viral infection such as the common cold or flu. Nitrofurantoin has been in extensive clinical use since 1952 and its suitability in human pregnancy has been well documented. However, as with all other drugs, the maternal side effects may adversely affect course of pregnancy. The drug should be used at the lowest dose as appropriate for a specific indication, only after careful assessment. Comment. The prolonged incubation period to onset of liver injury due to nitrofurantoin frequently leads to mistaken or delayed diagnosis. The clinical pattern is more similar to chronic than acute hepatitis and recovery may be delayed. Reversible interstitial lung disease with prolonged use of nitrofurantoin: Do the benefits outweigh the risks? Singh A(1), Singh P, Sidhu US. Author information: (1)Department of Pulmonary and Critical Care Medicine, Dayanand Medical College and Hospital, Ludhiana, Punjab, India. By contrast, despite over 50 years of use, resistance to nitrofurantoin does not appear to be increasing. This, coupled with improvements in the formulation, to minimize gastrointestinal side‐effects, has led to its recommendation as the sole first‐line agent for uncomplicated UTIs by the Dutch College of General Practitioners. Purpose. To review the current literature on reported pulmonary, liver, and nerve adverse reactions (ARs) of long-term Nitrofurantoin (NF) suppression in older patients treated for urinary tract infections (UTIs).
